At Bones 4 Life Chiropractic, we understand that your health information is personal. We are required by law to maintain the privacy of your protected health information (PHI) and to provide you with this notice of our legal duties and privacy practices.

2. How We May Use and Disclose Your Health Information

We use your PHI for the following clinical and administrative purposes:

  • For Treatment: We may use your information to provide, coordinate, or manage your chiropractic care. For example, we may share your PostureRay or APECS analysis with a referring specialist or your primary care physician.

  • For Payment: We use your PHI to bill and collect payment from you, an insurance company (such as Aetna or Medicare), or a third party.

  • For Health Care Operations: We use your PHI to run our practice and ensure you receive quality care. This includes internal audits, quality assessments, and digital tools like PhysiTrack for your home exercise programs.

  • Appointment Reminders: We may contact you via phone, email, or text to remind you of an upcoming visit or to share clinical blog updates relevant to your recovery.

3. Your Rights Regarding Your PHI

  • Access to Records: You have the right to inspect and copy your medical and billing records.

  • Amendment: If you feel the information we have is incorrect (e.g., an error in your Segmental Fixation map), you may ask us to amend it.

  • Accounting of Disclosures: You may request a list of certain disclosures we’ve made of your PHI for purposes other than treatment, payment, or operations.

  • Restriction Requests: You have the right to request a restriction on the PHI we use for treatment or payment.

4. Our Responsibilities

  • We are required by law to maintain the privacy and security of your PHI.

  • We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information.

  • We must follow the duties and privacy practices described in this notice and give you a copy of it.

5. Complaints

If you believe your privacy rights have been violated, you may file a complaint with Bones 4 Life Chiropractic, LLC or with the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.
