Your First Chiropractic Visit: Clear Answers, Thorough Care
If neck pain, back pain, headaches, stiffness, injury-related discomfort, or desk-work strain is affecting your day, your first visit should help you understand what is going on and what to do next.
At Bones 4 Life Chiropractic in Gaithersburg, Dr. Keith Murray provides individualized, education-first care. Your visit includes time to discuss your symptoms and goals, a focused evaluation, and a clear explanation of options that may be appropriate for you.
💡 NOT A CRACK-AND-GO APPOINTMENT
We do not use predetermined “forever” treatment plans. Recommendations are based on your symptoms, health history, examination findings, functional goals, response to care, and personal preferences.
Your appointment is designed to provide clear information and a reasonable path forward. The exact evaluation depends on your symptoms, health history, and individual functional goals.
Step 1: Tell Us Your Story
Your visit begins with a conversation about what brought you in. We discuss when symptoms began, aggravating/relieving factors, prior injuries, work demands, sleep, exercise, stress, and activities you want to return to. This helps Dr. Murray understand the full context of your concern—not just where you feel pain.
Note: Please arrive 15 minutes early if you plan to complete new patient paperwork in the office.
Step 2: One-on-One Clinical Consultation
Dr. Murray reviews your personal goals and clarifies how symptoms impact your daily activities. Whether you are dealing with an acute injury, persistent stiffness, headaches, or desk-work fatigue, the goal is to understand your functional limits.
Step 3: Focused Physical Examination
Targeted to your specific complaints. May include assessments of:
-
Range of motion and mobility limits
-
Muscle strength, endurance, and tenderness
-
Joint movement and palpation sensitivity
-
Sensation and reflexes when clinically appropriate
-
Balance, coordination, and functional tasks
-
Workstation, lifting, and activity postures

Step 4: Posture, Movement & Ergonomic Assessment (When Appropriate)
For desk workers or posture concerns, evaluations may incorporate objective digital posture photogrammetry (APECS) and ergonomic analysis (monitor height, chair setup, phone habits, and micro-breaks).
Clinical Note: A posture image does not diagnose a medical condition on its own; it is interpreted alongside your full exam.
Step 5: Imaging or Referral — Only When Clinically Indicated
Not every patient requires X-rays. Imaging is recommended only when red flags are present, trauma is suspected, or findings would alter the clinical management plan. In uncomplicated acute low-back cases, routine initial imaging is typically not advised. If specialty medical evaluation, MRI, or physical therapy is indicated, Dr. Murray coordinates the appropriate referral.
Step 6: Review of Findings & Transparent Care Options
In most cases, you will receive your first adjustment during this visit. Using precise, controlled techniques, we work to restore alignment to your spine. Many patients report an immediate sense of "lightness" or relief following their first session. Before any care begins, Dr. Murray reviews findings, contributing factors, and treatment options. You have time to discuss benefits, limitations, self-care exercises, and clear financial policies. You are never pressured into a pre-set care plan.
Step 7: Care May Begin at the First Visit
If clinically safe and appropriate, treatment or active movement guidance can begin on Visit 1. In other situations, time may be needed to review records, obtain imaging, or establish co-management before hands-on treatment starts.

Frequently Asked Questions
Question
How long is a first chiropractic visit?
Will I receive an adjustment on the first visit?
Do I need routine X-rays before being treated?
Do I need a doctor's referral to schedule?
What happens if chiropractic care is not the right fit for my condition?
Answer
First visits are longer than follow-ups because they include a full history, consultation, focused physical exam, and discussion of findings. Please allow approximately 45–60 minutes.
If clinically safe and indicated based on your examination, care may begin on Day 1. If your presentation warrants imaging or medical co-management first, hands-on treatment will be deferred until safety is confirmed.
No. Routine baseline X-rays are not medically necessary for all conditions. Imaging is ordered only when red flags, acute trauma, or specific diagnostic indications suggest it will alter your clinical management.
Direct access allows most patients to schedule directly without an outside physician referral. Individual health insurance plan requirements vary, so check with your carrier if you are seeking reimbursement.
If examination findings suggest your symptoms are outside the scope of conservative chiropractic care, Dr. Murray will explain the findings and connect you with a qualified medical specialist, imaging center, or physical therapist.
